New Client Form
In Pennsylvania, a massage therapy intake form is a legal requirement. Outside of that, I use this form to personalize your session.
Please print my intake form and bring it to your first session. Otherwise, I will provide it on your arrival (arrive 15 minutes early for your completion and my review).
Conduct
I hope that you can agree with my expectations of conduct while we work together. What I expect from my clients may not be the same as the location where you receive my service.
Age
You must be 18 years old to schedule or receive any of my services.
Arrival
Arrive 15 minutes prior to your scheduled appointment time to fill out my New Client Intake Form and for my review.
Late Arrival (Please Contact Me)
If you do not contact me before your scheduled appointment time, I may schedule another client and you will be considered a no-call no-show; otherwise, your session will end at the scheduled time and my full fee will be expected.
Reschedule or Cancellation
Please reschedule your session if you are sick.
If you contact me 24 hours before your scheduled appointment time: no fee.
Less than 24 hours: 50% of full fee for session scheduled.
Consistent occurrence: full fee for session scheduled.
No-Call No-Show
Full fee for session scheduled.
Inclement Weather
If you cancel your session because of inclement weather: no fee as long as you reschedule your session or the National Weather Service issued a storm warning either during the day or in advance of your scheduled session.
Payment
Payment is required at the end of your session.
Returned checks: $30 fee in addition to your session fee.
Privacy
Your records are kept in the strictest confidence and in a secure place. I am the only one with access, and they will not be released to third parties, including health care providers and insurance companies, without your written consent. I may surrender your records if required by law, but not without first informing you.
Inappropriate Behavior
Inappropriate behavior will not be tolerated by any client. Such as suggestive comments, sexual innuendos, or touching in an inappropriate way will result in the termination of all sessions. Full fee for the scheduled session will be expected.
Medical Condition
It is your responsibility to keep me informed of any current medical treatment and any changes in your health condition.
Visitors During Session
You may not bring children that will be left unsupervised by an adult during your session. I will not be responsible for the care of children.
Cell Phones and Music Players
Unless otherwise approved by me, cell phones and music players must be turned off before entering your session.
Tipping
Tipping is not required, but 15 to 20% of your scheduled session fee is customary and greatly appreciated.
